Evanesce Experience — Program FAQ
What is the Evanesce Experience?
The Evanesce Experience is a limited-time program designed to drive clinical
trial and
gather real-world feedback on the Evanesce family of composites. Eligible
clinicians will
receive a payment in exchange for completing a product evaluation
survey
after
purchasing, using and evaluating their experience.
How do I qualify for the program?
To be eligible, you must:
- Purchase $600 or more of eligible Evanesce products on a single invoice
- Apply the promo code EVX at time of purchase
- Register within 30 days using your invoice and contact information at theevanesceexperience.com/register
- Complete and submit the Evanesce Experience survey within 30 days of registration.
Where do I buy the products?
Eligible Evanesce products must be purchased from Henry Schein
Dental. Only
invoices dated within 30 days of the registration date will be accepted at
time of
registration.
Does the $600 minimum include shipping?
No. The $600 minimum must be met before shipping.
Promotions such as Buy 1
Get
1 on Evanesce product would make the invoice ineligible.
Do I need to use the promo code?
Yes. The promo code EVX must be applied at the time of
order. Orders without
the code
will not be eligible for the program.
How long after submitting the survey will I receive my check?
Checks will be mailed within 4-6 weeks after your completed survey has been
submitted and verified.
Where do I submit my invoice and survey?
Invoices must be uploaded at time of registration on
theevanesceexperience.com/register and
are required to complete your
submission.
Once your registration is verified, you'll receive access to the product
evaluation survey
which will automatically submit upon completion.
What do I receive for participating?
Upon verified completion of the product feedback survey, you will receive a
payment of
up to $600 in exchange for your time and insights. This is
not a rebate, but
compensation for your clinical evaluation and feedback in the form of $200
per
evaluation, maximum 3.
Can I participate more than once?
No. One clinician per Henry Schein account # is eligible for
participation
in the
Evanesce Experience. Multiple registrations under the same account number
will not be
eligible.

Why aren't Vermont and Minnesota eligible for the Evanesce
Experience?
Due to state-specific regulations around promotional healthcare programs,
we're unable
to offer the Evanesce Experience to participants in Vermont and Minnesota.
We're
committed to remaining compliant with all applicable laws.
Are promotions and discounts allowed?
Discounts are allowed, but promotional offers are not. To qualify, your
invoice total must
be at least $600 after any discounts are applied. Invoices
that include
promotional
product (e.g., free goods) will not be eligible.
