Registration & Eligibility

The Evanesce Experience is a limited-time program designed to drive clinical trial and gather real-world feedback on the Evanesce family of composites. Eligible clinicians will receive a payment in exchange for completing product evaluation surveys after purchasing, using, and evaluating their experience.

No. The $600 minimum must be met on a single sales order. Purchases across multiple sales order cannot be combined to reach the qualifying threshold.

Unfortunately no. Registration must be completed within 30 days of your qualifying sales order date. Sales orders outside this window will not be eligible for the program.

Please check your spam or junk folder first. If you still cannot locate your confirmation email, please contact us at support@theevanesceexperience.com with your name, practice information, and sales order details and we will look into it for you.

No. Participation is limited to one (1) per private practice account. Multiple registrations submitted under the same account number will not be eligible.

No. The Evanesce Experience is open to private practice accounts only. DSO and special market accounts are not eligible to participate.

The MPa Universal Free Good

Yes. The MPa Universal will ship with your Evanesce order as part of the same fulfillment.

No. The $600 minimum must be met by eligible Evanesce products only, before shipping, and excludes the value of the MPa Universal free good.

Yes. Promo code EVX must be applied at the time of purchase to receive the MPa Universal at no additional charge. Orders without the code will not be eligible for the program.

Discounts are allowed. Your sales order total must be at least $600 after any discounts are applied. The MPa Universal received through promo code EVX is the only free good permitted on a qualifying sales order.

Evaluations

Evaluations must be completed within 30 days of your approved registration. Evaluations submitted outside of this window will not be accepted and payment will not be issued for missed evaluations.

Each evaluation is designed to capture your real-world clinical experience with Evanesce. We encourage you to complete each evaluation after using the product in a clinical setting. All three evaluations must be completed within 30 days of your approved registration.

Yes. Payment will be issued for each evaluation completed individually at $100 per completed evaluation. You are not required to complete all three to receive partial payment.

Each evaluation survey is designed to be completed in approximately 10 to 15 minutes.

Each evaluation automatically submits upon completion and you should receive a confirmation email from support@theevanesceexperience.com. If you did not receive a confirmation, please contact support@theevanesceexperience.com with your name and registration details and we will verify your submission status.

Invoices must be uploaded at the time of registration at theevanesceexperience.com/register. Once your registration is approved, you will receive access to the product evaluation surveys which will automatically submit upon completion.

Payment

Upon verified completion of each product evaluation survey, you will receive $100 per completed evaluation, up to a maximum of $300 for three completed evaluations, issued via check to the name and address provided at registration.

Checks will be issued to the name provided at the time of registration. Please ensure the name entered at registration reflects how you would like your check to be issued, as changes after registration may not be possible.

If you need to update your mailing address after registration, please contact support@theevanesceexperience.com as soon as possible. Address changes cannot be guaranteed once payment processing has begun.

A check for $100 per completed evaluation, up to a maximum of $300, will be dispatched within 60 days of evaluation completion and verification.

If you have not received your check within 60 days of evaluation completion, please contact support@theevanesceexperience.com with your name, registration details, and mailing address so we can investigate and reissue if necessary.

Please contact support@theevanesceexperience.com with your name, registration details, and a summary of the evaluations completed. We will review your account and resolve any outstanding payments.

Yes. As required by the Patient Protection and Affordable Care Act of 2010, Clinician's Choice will report any payments made to you under this program to the Centers for Medicare & Medicaid Services (CMS).

Due to state-specific regulations around promotional healthcare programs, we are unable to offer the Evanesce Experience to participants in Vermont and Minnesota. We are committed to remaining compliant with all applicable laws.

Returns & Refunds

If a return brings your eligible Evanesce products below the $600 minimum, your eligibility may be affected. Returns related to qualifying purchases will be reviewed on a case by case basis. Please contact support@theevanesceexperience.com as soon as possible.

If a backorder results in your order being fulfilled across multiple invoices, eligibility will be reviewed at the sales order level. If your total eligible Evanesce product purchases on a single sales order meet the $600 minimum, your registration will still be considered for approval.

General Program

No. Limit one (1) offer per private practice account. Multiple registrations submitted under the same account number will not be eligible.

Survey feedback may be used in aggregated, anonymous form for the development of marketing materials and product claims. Individual respondents will not be identified by name or any other personally identifying information in any marketing collateral unless express consent has been provided.

You may opt out of having your testimonial or survey responses used for marketing purposes by emailing support@theevanesceexperience.com prior to survey submission. Opt out requests submitted after survey completion cannot be guaranteed.

Please contact support@theevanesceexperience.com as soon as possible with your updated information. Changes cannot be guaranteed once your registration has been verified and processing has begun.
